The discipline of turning a coordination problem - a clash, a missing piece of information, a design conflict - into a tracked item with an owner, a status, and a resolution, instead of letting it live only in someone's meeting notes or memory. Issues are typically exchanged between teams as BCF files, or managed directly inside a CDE platform, and a well-run project reviews open issues on a fixed cadence until the count trends to zero.

The whole point of running clash detection and model coordination is worthless without disciplined issue tracking behind it - finding a hundred problems means nothing if forty of them quietly never get fixed.
